View the TEFAP income guidelines to determine if your household is eligible (no proof required). If you fall within the guidelines for your household size, print the TEFAP application, complete, either scan or take a photo, include all household members’ names, birthdates, and age group, sign and email it to lakelandpantry@gmail.com. Please provide a copy of your ID for the TEFAP application. If we need further information, we can obtain it when you visit the pantry such as proof of address if not already included on your ID.

Completing forms in advance will hasten the registration process when you come into the pantry. The Ethnicity form allows us to apply for other grant money and no names are shared. The proxy form is for if you would like to have someone you designate to pick up your food.
